Protected Health Information

Do not send Protected Health Information (PHI) by email. Email is not a secure channel. Do not include patient names, dates of birth, insurance identifiers, account numbers, clinical details, or any other PHI in a reply to this message. All PHI must be transmitted only through the Platform, an approved EDI or SFTP connection, or another channel authorized in writing by RSAI.

Security

RSAI will never ask you for your password, one-time code, or authenticator seed by email, phone, or text. Sign in only at https://portal.revenuestream.ai. If a message asks you to sign in somewhere else, or requests credentials or banking details, do not respond — forward it to [email protected].

Electronic records and signatures

The agreement referenced above is presented and executed electronically. Electronic signatures and records are legally binding under the federal ESIGN Act (15 U.S.C. § 7001) and applicable state Uniform Electronic Transactions Act provisions. Before signing you will be asked to consent to transact electronically; you may decline and request an alternative method by contacting [email protected] before you sign. To access, review, and retain the agreement you will need a current web browser, an active internet connection, an email account, and the ability to view and save PDF files. You may download and retain a copy of the executed agreement from the portal at any time, and may request a paper copy at no charge. Only the individual designated by your organization as its authorized signatory may execute the agreement, and that individual represents that they hold authority to bind the organization.

Notices

Under Section 13.6 of the Unified Customer Agreement, email is a designated channel for delivery of contractual notices. Keep your registered contact address current in the portal, and add [email protected] to your safe-sender list — notices are effective on delivery whether or not you open them.

Confidentiality

This message and the documents it links to are confidential and intended solely for the named recipient. The agreement contains RSAI pricing, fee, and transaction-rate terms that are proprietary commercial information and trade secrets of RSAI, and that may not be disclosed to any third party without RSAI's prior written consent. If you received this message in error, delete it and notify [email protected]; any review, use, or distribution is prohibited.
